Developing a Personal Care Plan

As soon as possible after you move in to our Care Home we put together your very own personalised Care Plan which is agreed by you. This is prepared in collaboration with you or your family, and is based on your needs as we have assessed them. The Care Plan includes information about you as a person as well as about your medical, physical and personal needs. It ensures that the care you receive is consistent, continuous, and delivered in a way that respects you as an individual – taking into account not just your physical needs but your personal preferences and interests too. As these may change from time to time your Care Plan is reviewed monthly and updated as necessary to ensure you always receive the level of care that is most appropriate for you. Your Care Plan is looked after by your named nurse and securely locked away to ensure that personal information about you or your health remains confidential.
